We will be using “DropBox” for our data collection to make the registration process as simple as possible. If you are a Team Manager or Assistant Coach and need access to DropBox please contact your Coach.

PROGRAM ADMINISTRATOR REGISTRATION (Expires July 31)
California Youth Soccer Association – South (CalSouth) – Program Administrator
[Rule 6.3.1.2.1 Program Administrators are defined as: State Board, Coaching Education staff, O.D.P. coaches and scouts, district, league and club officers, program directors, team managers, athletic trainers, coaches, assistant coaches, and substitute coaches, and anyone who has an official capacity in the soccer program.]

1.  Cal South Program Administrator Registration Form
Open with Adobe Reader so you can save and print the document.
a) Have your administrators complete this form using Adobe Reader.
b) Upload a signed form for each admin to your Dropbox folder.
c) Obtain three (3) signed copies

2. AYSO Registered Volunteer
a) Upload a form for each admin to your Dropbox folder.
b) Obtain three (3) signed copies

3. Administrator Photo – required for Administrator Passes (ID Cards)
a) Take a picture of each Administrator.
b) Head shot with no head gear (hat, visor, headband, …) and no sunglasses.
c) Minimum 160 x 160 pixels about 50KB
d) Save the file (First Name, Last Name, Gender/Division, Coach last Name, Year) for example “Alex Ferguson Bu12 Klinsmann 2013″
e) Upload the photo to your Dropbox folder.

4. Live Scan (CalSouth Risk Management) – One time only

5. Concussion Awareness Certification (CalSouth)

Complete the online course. Concussion Certification www.calsouth.com/en/safety/
SAVE A COPY OF THE CERTIFICATE !!!
CalSouth and AYSO both use the same certification so you may want to send a copy of the certificate to your AYSO Registrar and request that they update your AYSO Record.

6. Coach and Assistant Coach are to be USSF License (Cannot accept foreign licenses).
